We look forward to becoming your dental provider. For optimal dental health, we recommend that you have a complete dental exam and cleaning once every six months. This ensures that your teeth are well maintained and any problems are spotted early, before they become of significant concern.
What to Expect
Please arrive 10-15 minutes early to fill out necessary paper work. You can save time by downloading the Patient Forms below, complete them and bring them with you to your first appointment. This will assist our dental providers in becoming familiar with your medical and dental history and help us to provide you with the best, personalized care possible.
Printable New Patient Forms
Our New Patient Forms are now available for download so that you can complete your form at your convenience. This gives you the time you need to carefully review our policies and procedures outside of the office and makes your first visit easy. Click on the link below to download the form, print it out and bring it to your first appointment.

What Else You Should Bring With You To Your First Dental Appointment:
- Records and/or x-rays from previous dental providers
- Dental insurance card
- The co-payment required by your insurer – we take cash, check, and credit cards
- A list of the medications you take
If You Have Recent X-rays Taken By Another Dentist
We do accept x-rays from other dentists as long as they are of diagnostic quality and are current. Bitewing x-rays must have been taken within the past year, and Panoramic x-ray within the past five years. We kindly ask that you complete the General Dental Release form in order for your records to be released to our office from your previous dental provider. Please sign and send this form directly to your previous dentist.
